My fiance is sore after . What can we do?

Not really sure what to do about this. My fiance has had a child and since then she becomes sore after . The thing is, she's only sore where she had her episiotomy. We've looked online at other sites and there hasn't been much help. Her baby is now 19 months old, so that's how long this has been happening. We've tried lubricants, and a bath after but nothing seems to prevent the soreness. Can anyone suggest anything to help?
